The tourists coming to India must fulfill 2 different forms on their arrival:

Arrival card — the form which you should complete on your arrival to the airport of India and then show this document to the Customers office representative at your entry into the country.
Departure card — the form which you should complete on your arrival to the airport of India and then show this document to the Customers office representative at your departure from the country.

1. Name.

This information field must be completed with capital letters only. Enter the name and surname in accordance with those written in your international passport.

2. Sex: Male or Female.

3. Nationality.

Write your nationality with capital letters only.

4. Date of birth.

Your date of birth should be written as following: DAY/MONTH/YEAR

5. Country of residence.

Specity the country of your permanent residence with capital letters only.

6. NRI/PIO/OCI status. Put NONE in this information field.

Comments: NR = Non Resident Indian

PIO = Persons of Indian Origin

OCI = Overseas Citizen of India

7. PIO/OCI card No — leave empty.

8. Passport Number.

Write here the number of your international passport.

9. Date of issue.

Write here the date of issue of your international passport.

10. Flight Number.

Here you should specify the number of your flight.

11. Port of boarding: Put here DABOLIM.

12. Date of arrival.

Write the date of your arrival to India.

13. Countries visited last 6 days.

In this information field you should specify all countries you have visited within last 6 days of your travelling.

14. Addres s in India.

In this information field you should write the address of your stay in Goa (for example: Goa, Anjuna resort, Guest house “Polo”).

15. Telephone No.

Write your standing phone number.

Fields 16, 17, 18. — leave empty.

19. Visa number

Write the number of your visa you have in your international passport.

20. Date of expiry.

Write the date of your visa expiration as follows: DAY/MONTH/YEAR

21. Type of visa — put “Tourist” here.

22. Expected date of departure.

Write the date of date of your supposed departure as follows: DAY/MONTH/YEAR

23. Purpose of visit — put “Holiday” or “Vocation” here.
